  Assess, plan, implement, and evaluate individualized patient care plans
  Collaborate with physicians, patients, families, and interdisciplinary healthcare teams
  Promote patient safety, health education, and positive clinical outcomes
  Coordinate care transitions and discharge planning
  Delegate and supervise appropriate patient care activities
  Maintain accurate documentation and adherence to clinical standards
